The app (and steam) automatically shuts off when you unplug the headset sometimes. The screen blacks out because it loses power. The reason you still hear audio is because your HDMI works fine.

Because what you're experiencing is the USB disconnecting. My last idea is my PSU (xfx 550w) is dying or something, or maybe i'm too close to the sensors? I don't have IR sources close to the playing area except.the sun coming from the window (but problems happen during the night too), I have a big mirror right in front of the sensors but I always cover it with a blanket during each session.
Sometimes the problem is worse, sometimes it's less evident, sometimes it doesnt reboot at all even after hours of playing sessions, sometime it does it quite often, I don't know what's the problem but to me it doesn't look like the Rift is faulty or anything, seems more like something related to my PC or a software issue. I have a i5 6500, a gtx1070, 16 gb of ram and all the games are on a SSD so I should be fine with my specs, at least in the rift menù. I also bought the Inateck pci card and plugged the sensors there, leaving the rift on my motherboard ports but nothing changed. I thought I had some loose cables but it just happens randomly, I tried energically moving the cable near the motherboard hdmi plug and on the rift side but I never managed to trigger the turning off thing. I can still unfreeze it by pressing the menù button, this brings me back to the working white menù, but I'm forced to quit cause I can't play if I don't reboot the game. Unfortunately when it turns on again, the menù "freezes" if I try to go back to the game. When the reboot happens, sometimes I can still hear the game through the rift headphones cause the application keeps running. When it happens, usually the Oculus app reboots too. On top of that, it often "turns off" like it's doing a reboot, the screen turns off and the little led inside the rift becomes orange (or red, dunno i'm slightly colorblind lol). I'm experiencing some problems with my Rift tracking: it goes smooth and nicely for few seconds but then I have "jumps" in my headset tracking (touch tracking seems ok or at least it's better), like it loses my headset for a fraction of a second.
